Question 21 on the SF 86 (National Security Clearance Questionnaire) asks whether or not in the last 7 years you have had mental health counseling. Grief, family, and combat trauma counseling are excluded from disclosure, but sexual assault counseling is not.
Servicemembers seeking security clearances are required to complete this form. The presence of Q21 discourages many servicemen and women from seeking desperately needed counseling following a sexual assault, as they are concerned about either losing their clearance or disclosing the rape to their chain of command.
