Do away with fining people who are so poor that they cannot afford health insurance and can't get Medicaid due to income

I would like to see the fine for lack of health insurance be done away with because some people cannot afford it even with the government paying 80% of the premium and cannot get Medicaid because their income is too high to qualify for even Affordable Care Act Medicaid. I would also like to see Medicare be mandated to include vision, dental and hearing in their coverage as good dental, vision and hearing is part of our overall health and is equally important.
