eliminate exploitative medical insurance tactics

Require the elimination of predatory practices by medical insurance plans that add costs and reduce benefits
for the insured such as:
--increasing the insured's fee/dues substantially and repeatedly after claims have been made
--not applying to deductibles and yearly maximums many of the insured's out-of-pocket expenses, such as for doctors and laboratory and imaging tests
--burying excluded or reduced benefit footnote lists in the plan's evidence of coverage or in obscure areas of the plan's website, separated from the main benefit descriptions
--not making easily available--before signing up--a plan's complete 'evidence of coverage'
These tactics by medical insurance companies constitute at best a lack of transparency, and at worst bait-and-switch practices.
