There should be a nation wide in-car driving test for anyone over the age of 65 bi-annually. This testing by the DMV would be the same as for a 16 year old new driver, including highways, city roads, and parking ability.
When an adult is rendered unable to drive safely any longer; the family would be contacted along with the Council on Aging to provide the person(s) with travel assistance to keep them mobile safely. This test would be free due to the limited income of the elderly. If the elderly can provide a safe car that they cannot drive it is to be donated to a service that will be providing them transportation, then in return creating more jobs for people providing these types of service already.
