There is a great need for the Department of Defense and the Veterans Administration to coordinate and combine their medical records. Often a veteran must use DOD medical services that are not available within the VA outpatient services, yet instead of one medical record for one patient, there are two, one maintained within the DOD healthcare system and one maintained in the VA healthcare system. Often a DOD healthcare provider cannot access vital information from the VA and vice versa. The current medical records system greatly increases the risk of medical errors.
