Put forth on the next general election ballot a constitutional amendment for Congressional Reform

To put forth on the next national ballot a Constitutional Amendment to limit the terms, compensation, and benefits of Congress.
To:
Limit terms of Senators to two term limits of 6 years each.
Limit terms of Representatives to six terms of 2 years each. No Congressman or Senator shall serve terms totaling more than 12 years.
Once the limit is reached, eliminate the lifetime salary benefit.
Hold Congress to the same health care programs it expects the people to participate in .
Require Congress to contribute to their own retirement in the form of a 401K with the Federal Government making contributions ONLY while serving in office.
Require Congress to be reliant on Social Security under the same plan as the average American Citizen and require contributions to same.
