Immediate Action Requested to Support & Enforce all Home Health Agencies with Case Management and Nursing Services for the disabled children. Case Management Services: 1. Assist Patients with Community Resources. 2. Assist Patient with Durable Medical Equipment and Supplies. 3. Advocate for Patient. 4. Administer Nursing Orientations 5. Provide Case In Services for Nurses. 6. Do Frequent Home Visits. 7. Become Involved in Patient-Parent-Physician Conferences. 8. Assist with Durable Medical Equipment & Supply Vendors. 9. Provide Case Educational Materials. 10. Keep Patient Home Health Chart in Order. 11. Become Available As Back Up Staff. Nursing Services: 1. Provide High Tech Nurses. 2. Complete All Case Required In Services. 3. Have Strong Team Skills 4. Be Reliable, Ready and Responsible
