Medicare requires Providers to have Patients fill out a Medicare Secondary Payer Questionnaire. Providers and Patients do not understand questions 7 -12. Simple request as 7-12 questions do not make sense and need to be changed. Mayo Clinic alone hands out over 100,000 of these a year. It is time consuming and distracting at a tough time in every Patients life.
