Simplify the home care regulation requiring a F2F form

The affordable care act has many welcome items but one new regulation is causing an undue burden on home care. In order to get paid we must receive a form with duplicate information on it including a narrative from an MD saying the pt was seen in the last 90 days and why home care is needed why the pt is home bound and what services are needed this has to be signed on a specific form and duplicates what is on the form that has always been needed. The 485. This overly burdensome requirement is taking needed resources away from pt care Could be met with a shorter addendum to the 485 that says " I saw this pt on this date and they need home care". The 485 already has what is needed and why. Stated intent of the law was for MD to see the patient in person before/after ordering service
